Rebecca 1   R and D started care with me in the second trimester of their third pregnancy.  R had two previous cesareans and was seeking a provider who would allow her a chance for a vaginal birth after two cesareans. Things went along perfectly.  I had vacation dates scheduled for March when R hired me, but we thought all should be well.  On March 24th, two days before I would be back home, R sent me a text that there was a storm in the area and she was having contractions strong enough to wake her.  I gave her some recommendations and after a couple hours the contractions slowed to every 15-30 minutes.  Things continued the same way that day.  The next morning R sent a text saying contractions were very irregular but much stronger and she was passing some show.  I recommended she touch base with my back up midwife so she knew what was going on in case she was needed.  Contractions shifted between 5-15 minutes apart all through out the day.  That night I was sure I would wake up to a text that R had birthed her baby, and I was so sad at the thought of missing!! I woke up the morning of March 26th and there was no baby born!  I was so relieved.  I would arrive back in Houston at 6pm and was very hopeful things would hold out for me!  Irregular, strong contractions continued through out the day.  After speaking with R, we made the plan for her to meet me at my home at 8pm to do an exam and see what was going on. R and D arrived at 8pm.  Contractions appeared to be 10-12 minutes apart while at my house.  After talking over options, R decided she was open to me sweeping her membranes and seeing if that would bring her into active labor.  Her cervix was 4cm dilated, 100% effaced and baby at a -1 station.  I swept her membranes and was very hopeful R would be calling me in the night in active labor. I arrived at R’s birthing location a little before 1am, after receiving some updates from R’s sister that indicated it was time for me to come.  I walked into the room and found R laboring on her birth ball, her doula at her side providing counter pressure.  Contractions appeared to be coming every 2-3 minutes, lasting 45-60 seconds long and strong in intensity. Rebecca 2 R was open to a cervical check and quickly moved to the couch after a contraction.  She was now 6cm dilated and baby at a 0 station.  Her cervix was very stretchy and I had no doubt that things would be moving along fairly quickly.  R was ready for the birth pool, so we helped her get comfortable in the pool.  She kneeled in the pool, rocking her hips and moaning gently during contractions.  Her support team took turns holding her hands and providing counter pressure to her back. Rebecca 3 Right before 2am, R informed the room that her water had just broken.  I listened to the baby’s heart beat and baby sounded perfect.  I grabbed a pair of gloves and was ready for when R felt primal and pushy. Within a few minutes R exclaimed she was feeling a lot of pressure.  Within a couple labor waves that pressure became stronger.  At 2:10am, R was dilated to 9.5cm and baby was at a +1 station.  After discussing, I held back the lip of cervix while R pushed.  After two contractions, the cervical lip was over the baby’s head.  R’s doula reminded her that she wanted to try to catch the baby, so R changed her position to semi reclining so she would be in a more optimal position for catching her baby. Soon we could see a small amount of the baby’s head.  R would feel for her sweet baby’s head as well as looking with a mirror at the progress.  She felt so encouraged as she could see and feel her sweet girl moving closer to the outside. Rebecca 4 The head was sitting on the perineum at 2:31am.  A couple more pushes brought the head to crowning.  D was at my side and ready to help catch his sweet girl.  One more push brought the head and the body soon followed into her daddy’s waiting hands.
